Voluntary Networks of Specialty Care Providers
(NOT available to SuperMed PPO Network members)
GPS is pleased to be a partner with three voluntary networks of providers. These networks provide high quality care at reasonable costs. These networks allow you and your family to obtain necessary care, while minimizing out-of-pocket costs. The use of these networks will help reduce the need for premium increases as these increases are a direct result from claim dollars paid by GPS.

FREQUENTLY ASKED QUESTIONS
Containcare
Most likely, no. Under most plans this is a voluntary network. You will still need to use an in-network provider. Using the Containcare providers will reduce health costs.
When your physician prescribes an MRI, CT, and/or P.E.T. scan, all you have to do is call Containcare at 800-331-5720. They will schedule your procedure in a facility that is convenient for you. Containcare will also notify your physician of the date and time of your test and even call to remind you of your appointment and answer any questions you may have. Your physician will then receive your results in a timely manner.
LAB CARD SELECT
Most likely, no. Under most plans this is a voluntary network. If you choose not to use Lab Card Select, you still have your PPO network.
Outpatient lab work that can be processed under Lab Card Select includes: blood tests, urine tests, cytology and pathology (e.g., pap smears, biopsies), and cultures.
There may be an approved collection site in your area. Check the website at
www.labcardselect.com, or call 1-800-750-1253 to see if there is an approved collection site in your area. If a collection site that meets your needs is available, you can take a completed test order from your physician outlining the tests to be performed to the collection site.
