Before you try to find a PPO provider, refer to your ID card to find your network. Click on your PPO network logo below to begin your search. Your PPO network also includes access to Mayo Clinic in Rochester, Minnesota, as well as the Rochester Mayo affiliated hospitals.
Your ID card also lists Multiplan PPO. This is a network you can access for emergency care when you are traveling outside your regular network service area. Use of providers in the Multiplan network will still be considered Out-of-Network and paid at a reduced benefit level unless you meet the requirements for an Emergency/Travel exception.
